Aged Care Support at Home
We provide inclusive coordinated home care packages to those who are eligible. Please find details below of what this looks like.
Your Care Coordinator :
Your Rainbow Care Coordinator will work with you to design your supports to make the most of the budget available to you.
Your Support Budget :
The government will assess your support needs and assign you a budget to support this. Then you can nominate Rainbow Care to be your provider of choice to administer your funding.
If you do not believe your budget matches your level of need your care coordinator can work with you to request a re-assessment or explore alternate support pathways and funding.
Design Your Supports
Rainbow Worker Team
Rainbow Care has a team of qualified and experienced care workers to support your personal care, in-home and community needs.
Allied Health & Clinical Team
If your needs are little more complex we will work with you to set up clinical supports such as nursing, occupational therapy, physiotherapy, other allied health supports or work with your existing team.
Equipment, mobility Aids and Maintenance
From a walking stick to wheelchair, continence products, consumables, day chairs to home modifications. Your care coordinator can work with you to organise any needed equipment or aids, including referrals to allied health for recommendations.
Other Supports
Yard care, transport, emergency respite, meal delivery, home maintenance.
Monthly Statement & Budget Dashboard
You will begin receiving a monthly statement at the start of each month. This will show you how much funding you have, what has been used, by who and of course what you have left to spend. You can also have access or nominate somebody you trust to have access to our online budget dashboard to view any invoices processed and track your budget.
Preferred Provider
The choice is yours on who provides your supports. If you already have preferred providers like the lawn maintenance team that you have been using for the last 20 years or the physiotherapist down the road your care coordinator can work with you to have these providers set up to provide these supports under your funding package.

To access these services, you must have first been assessed and allocated an individual funding package. If you’re unsure but know you need the support let us know and we can walk you through the next steps. Note: We are not a CHSP provider
